CHECOTAH FITCLUB / MUSKOGEE FIT Membership Agreement, Facility Rules, and Liability Waiver
This agreement is entered into by {name} ("Member"), in consideration of being permitted to access and use the facilities and services of Checotah Fit Club ("CHECOTAH FITCLUB / MUSKOGEE FIT Facility"). This agreement outlines the terms of membership, liability waiver, and code of conduct.

All payments are billed in advance. Membership renews automatically unless canceled according to the cancellation policy below.
Please note: All payments made by debit/credit card or other non-cash/non-check methods will incur a 4% processing fee in addition to the listed amount.
2. Cancellation Policy
Members may cancel their membership at any time prior to the next billing cycle using the same method by which they enrolled (online, in-app, or in-person).
Cancellations must be submitted at least 24 hours before the next scheduled renewal to ensure processing. Cancellations received less than 24 hours in advance may not be processed in time, and the Member will remain responsible for the next billing cycle.
A cancellation confirmation will be provided via email.
It is the Member’s responsibility to initiate cancellation by using the cancellation option provided within the app or on the website. Verbal requests, messages, or non-use of the facility will not constitute cancellation.
3. Access & Usage

Member will receive app-based or key fob access.

Members must be 18 years or older to sign up for an individual membership. Minors under 18 may only join with a parent or immediate family member who also holds an active membership. The responsible adult must co-sign this agreement and will be fully responsible for the actions and conduct of the minor member.

Sharing access or intentionally allowing non-members into the facility is a serious violation and will result in an automatic $50 unauthorized guest fee. Repeated or severe violations may result in permanent termination of membership without refund.
4. Rules & Code of Conduct
Member agrees to follow all posted facility rules and those listed here:

Wipe down equipment after use.

Wear appropriate athletic attire (no jeans or muddy shoes).

No smoking, alcohol, drugs, or weapons allowed.

Treat others with respect; no harassment or aggressive behavior.

No unauthorized business, sales, or promotions on premises.

Facility hours, services, and rules may change at the discretion of Checotah Fit Club .
5. Medical Emergency Authorization
In the event of injury or illness, I authorize Checotah Fit Club staff to seek emergency medical care and share relevant health information with providers.
I accept financial responsibility for any resulting care. I acknowledge I am encouraged to carry personal health insurance.
6. Assumption of Risk & Waiver of Liability
I, {name}, understand that participation in fitness activities, including but not limited to personal training, weightlifting, martial arts, tumbling, boxing, kickboxing, yoga, and cardio classes, involves risk of physical or emotional injury, illness, paralysis, or death. I voluntarily assume all such risks.
I hereby release and hold harmless Checotah Fit Club , its owners, staff, trainers, affiliates, and representatives from all liability, claims, or demands related to my participation, to the fullest extent permitted by Oklahoma law.
This waiver extends to injuries from equipment, participation, or other members and includes all locations and events hosted by Checotah Fit Club .
7. Agreement and Signature
By signing below, I acknowledge I have read and fully understand the terms of this agreement, including cancellation, conduct, and liability waiver. I enter this agreement voluntarily and agree to abide by all policies set by Checotah Fit Club .
